a Pierre's product. |

Register your organization for qualification
using the forms below to demonstrate qualification.

Have the members of your organization start
collecting the UPC bar code from any product with the Pierre's name on
it (excluding loose novelty items) |
|
Send the UPCs in bundles of 100 to Pierre's,
Attn: Sweet Rewards Program, 6200 Euclid Ave., Cleveland, OH
44103. All shipping costs are the responsibility of the sender. Pierre's
cannot be responsible for lost, stolen or misdirected shipments. |
|
Minimum of 100 UPCs valued at $10.00 in a
calendar year. Maximum of 100,000 UPCs valued at $10,000.00 per calendar
year. |
|
Checks will be issued only to the name of
the officially qualified organization noted on the registration form
for the program -- no individuals. |
